Welcome to Community Roots Health (“we,” “our,” “us,” or “the Clinic”). By accessing our website, booking appointments, or using any of our telehealth or in-person services, you agree to the following Terms & Conditions. Please read them carefully.

1. Scope of Services

Community Roots Health provides:

  • Virtual primary care

  • Mental health support

  • Structured weight-loss programs

  • Cardiometabolic and chronic disease management

  • Remote patient monitoring (RPM)

  • Preventive care and wellness services

Our services are intended for non-emergency medical care.
If you are experiencing a medical emergency, call 911 immediately.

2. Eligibility for Care

To receive services, you agree that:

  • You are physically located in Florida during telehealth visits

  • You will provide accurate demographic, medical, and insurance information

  • You are at least 18 years old, or have a parent/guardian for minors

  • You will maintain updated contact information

3. Telehealth Consent

By receiving telehealth services, you acknowledge that:

  • Care is provided via secure audio/video communication

  • Your provider will determine if telehealth is clinically appropriate

  • Technical issues may occur and may affect your visit

  • You may withdraw consent to telehealth at any time

4. No Guarantee of Medical Outcomes

We use evidence-based guidelines, but:

  • Outcomes vary based on medical history, adherence, and individual factors

  • We do not guarantee specific results for weight loss, BP control, glucose control, or any treatment

5. Pricing, Payment, and Billing

You agree that:

  • Payment is due at the time of service unless covered by insurance

  • Self-pay prices (e.g., $60 primary care, $60 mental health, $80 weight loss) may change at any time

  • You are responsible for deductibles, co-pays, or uncovered services

  • Insurance coverage is not guaranteed

  • Denied claims are the patient’s responsibility

6. Prescriptions & Medication Policies

Medication prescriptions (including controlled substances, weight-loss medications, hypertension medications, or psychiatric medications) are:

  • Issued only at the provider’s clinical discretion

  • Dependent on accurate medical history and lab results

  • Subject to monitoring requirements

  • Not guaranteed with every visit

Misuse, diversion, or failure to follow medical instructions may result in discontinuation of care.

7. Weight-Loss Program Terms

By joining our weight-loss program, you understand that:

  • Lab work or ECG may be required prior to prescribing

  • Medication availability may depend on insurance & medical necessity

  • Adherence to diet, exercise, and follow-ups is essential

  • Weight-loss results vary by individual

8. Remote Patient Monitoring (RPM)

If enrolled in RPM, you agree that:

  • Devices may automatically transmit health data to our clinic

  • You will use equipment as instructed

  • You will report significant symptoms promptly

  • RPM is not a substitute for emergency care

  • Insurance may be billed monthly for monitoring

Devices damaged through misuse may incur replacement fees.

9. Appointment / Cancellation Policy

  • Appointments must be canceled or rescheduled 24 hours in advance

  • Late cancellations or missed appointments may result in fees

  • Repeated no-shows may result in dismissal from the clinic

10. Privacy & HIPAA Compliance

We protect your information in accordance with:

  • HIPAA

  • Federal and Florida privacy laws

Your data is used only for treatment, billing, insurance processing, and clinic operations.

11. Prohibited Conduct

You agree not to:

  • Provide false medical or personal information

  • Misuse prescriptions or request inappropriate medications

  • Abuse staff, threaten providers, or behave disruptively

  • Use our website for unlawful or fraudulent activities

12. Termination of Care

We reserve the right to discontinue care for:

  • Abusive or threatening behavior

  • Repeated no-shows or nonpayment

  • Medication misuse or diversion

  • Failure to follow medical recommendations

  • Fraudulent information

When appropriate, we will provide emergency coverage and referral.

13. Website Use

By using our website, you agree:

  • To use it for lawful purposes only

  • Not to attempt hacking, scraping, or disrupting functionality

  • Not to impersonate another person

14. Limitation of Liability

To the fullest extent permitted by law, Community Roots Health is not liable for:

  • Technical failures during telehealth visits

  • Patient misuse of medication or medical devices

  • Failure to follow medical advice

  • Delays caused by third-party vendors

  • Losses related to insurance denials

15. Changes to Terms

Continued use of our services means you accept the new Terms.
