Community Roots Health provides primary care/internal medicine and psychiatry in Palatka, Florida. We focus on longer, personalized visits, clear treatment plans, and proactive follow-up—so patients feel heard and supported. We care for adults of all ages, with a strong focus on Medicare and older adults, and offer telehealth when appropriate.
